for assert et al.
* kern/assert.h: Likewise.
* kern/debug.h: Likewise.
* util/debug.h: Likewise.
* i386/i386/debug.h: Move dump_ss definition out of [DEBUG].
* linux/dev/glue/block.c (rdwr_full): Remove invalid assert.
